Glaucoma is a leading cause of irreversible blindness worldwide, and glaucoma drainage devices (GDDs) play a crucial role in its surgical management. These implants, including the Ahmed Glaucoma Valve, Baerveldt implant, Molteno implant, and PAUL® Glaucoma Implant (PGI), facilitate aqueous humor drainage to lower intraocular pressure, particularly in cases of advanced, refractory glaucoma or failed filtering surgery. Despite their efficacy, GDD implantation is associated with complications, including corneal endothelial cell loss, which can lead to corneal decompensation. This chapter explores the impact of GDDs on corneal health, focusing on the mechanisms, risk factors, and management of corneal decompensation post-GDD implantation.
